It is advised that you read the description of all of the below forms and reports. This way you can ensure that you have all the supporting documentation to apply successfully.

This is the application form that all applicants must present at the time of their appointment. Read all of the questions carefully and fill it out to the best of your ability. Guidance notes are available if you're unsure about anything.

This report is required for those who hold a licence from a Recognised State . It is not required for those who hold a licence issued in the EU. Exception: If you previously wore glasses for driving, and have since had laser correction, you will need this report to prove that you no longer require glasses to drive.

The medical report is required for those who have a medical condition. This is outlined on page 3 of the application form. If you tick Yes to any of the medical questions you must submit this report.

If you hold a group 2 category such as a C(Truck) or D(Bus), you will need this report in order to exchange these categories. However, if you do not wish to exchange these particular categories, you can instead submit a Declaration to Surrender Categories form.

This declaration is only required if you do not wish to exchange your group 2 categories. This includes C(Truck) and D(Bus) categories.

If you have lost your licence or a part of your licence such as a Counterpart, you will need to submit this form.

You will need a Letter of Entitlement from your country's driving authority in place of your licence. This letter will contain details of your entitlements and any restrictions you may have.
